CUTC membership application guidelines:

1. Read Article III (Membership) of the CUTC Bylaws.
2. Complete the Application for Membership form.
3. Printout a hard copy of the Application for Membership form and return it completed with a supporting letter and documentation as defined in Article III of the CUTC Bylaws to:
